If your dandruff shampoo isn’t working and the flakes keep coming back, you’re not alone, here’s the thing: it’s probably not dandruff. We’ve all been there: a sprinkling of white flakes on your shoulders, an itchy scalp that just won’t quit. Most people’s first instinct is to call it dandruff and grab whatever anti-dandruff shampoo is on the shelf. Here’s the thing: it’s probably not dandruff. And if you treat the wrong cause, you can actually make the real issue worse by stripping a dry scalp further, or under-treating an actual yeast overgrowth so it just keeps coming back.
I want to walk you through what’s actually causing your flakes, how to tell the difference, and what I’ve found actually works, including the products and in-salon treatments I use with my own clients.

What’s Causing Your Flaky Scalp?
A flaky scalp isn’t one-size-fits-all, and treating the wrong cause is usually why people feel stuck in a flake-shampoo-repeat cycle for years.
A lot of flaking traces back to two microscopic residents everyone’s scalp already hosts: Malassezia yeast and Demodex mites. Both live quietly on a healthy scalp and cause zero problems, until sebum builds up and gives them too much to feed on. Malassezia metabolizes that sebum into irritating free fatty acids, which is the direct chemical trigger for itch and flaking. Demodex mites feed on the same buildup and, when overpopulated, can contribute to follicle irritation and inflammation. So flaking is rarely a hygiene failure. It’s usually a buildup problem that’s letting these two normal residents get out of balance. This is why dry-scalp treatments don’t touch dandruff, and dandruff treatments don’t touch a dry scalp.
Below, I’ve outlined the most common culprits, their causes, and the best treatments, including when my Scalp Rejuvenation Treatment might be the right call.
- Dandruff (Seborrheic Dermatitis)
- Cause: Overgrowth of Malassezia yeast, which feeds on excess scalp sebum and releases irritating free fatty acids, leading to inflammation and oily, yellowish flakes. Demodex mites, which also feed on sebum and live inside the follicle itself, are frequently part of the same imbalance and can compound irritation.
- Symptoms: Greasy patches, white or yellow flakes, and itchy redness.
- Treatments:
- Medicated shampoos with zinc pyrithione, selenium sulfide, or ketoconazole.
- The part people get wrong: If it really is dandruff, an actual yeast overgrowth, washing once a week with a dandruff shampoo isn’t going to fix it. Malassezia’s food source is sebum, and sebum doesn’t wait a week to build back up. You need to cleanse efficiently, effectively, and frequently enough to keep that food source at bay, not just occasionally knock it back. Once the yeast overgrowth is actually resolved, then I can shift focus to what your hair and scalp need after: hydration, repair, whatever comes next. It’s rare that anyone needs prescription-strength yeast control for longer than about six months; this isn’t usually a forever-treatment, it’s a reset.
- Why I’m picky about conditioning products here: The yeast that feeds on scalp oil doesn’t know or care where that oil came from. It doesn’t distinguish between your own sebum and an oily conditioning agent sitting on the scalp from a rinse-out conditioner (or even a typical shampoo) to Malassezia, it’s all just food. That’s exactly why I don’t recommend just any conditioning scalp treatment, nor just any shampoo, for a truly dandruff-prone scalp: the wrong one can hand the yeast more to eat and undo the work the cleansing shampoo just did. In the salon I do use a scalp conditioning step, but I’m deliberate about which one and when. It’s not a blanket “add conditioner” recommendation.

- Symptoms: Redness, swelling, flaking, and irritation.
- Treatments:
- Identify and discontinue irritating products.

- Scalp Folliculitis
- Cause: Infected or inflamed hair follicles, often bacterial or fungal. Demodex overpopulation is also a known contributor here, since the mites live inside the follicle itself.

- Hormonal Imbalances
- Cause: Androgen fluctuations leading to oiliness or dryness.
- Symptoms: Flakes linked to changes in scalp oil production.
- Treatments:
